Home Health Services - Under a Physician’s order, the Home Life Healthcare Team can provide one or more of the following services:

Skilled Nurses

A registered Nurse coordinates patient’s care, oversees progress and helps patients and caregivers understand:

  • Stages of Illness and Recovery

  • Use of Medication and Diet

  • Recognition of Symptoms in Recurring Illness

  • Treatment Procedures (i.e. wound care, oxygen therapy, care of indwelling catheters, etc.)

  • Patients Rights and Advance Directives

 

 

 

 

Chronic Disease Management

Our Chronic Care Management Program from Home Life Healthcare helps patients with chronic illnesses – such as diabetes, heart failure and breathing problems – to better manage their health and achieve the best quality of life possible.

Our skilled, certified nurses help patients in their homes learn ways to manage their illness. Through coaching and education, patients and families learn self-management skills that help them feel more confident to live a healthier lifestyle.

Heart Failure and CHF Program

At Home Life Healthcare, we offer comprehensive and personalized care designed to meet the needs of patients’ living with heart failure. We provide state of the art disease management interventions to reduce the frequency and duration of hospital visits. Post Joint Replacement Therapy

The goal of physical therapy after joint replacement surgery is to help restore you to your highest level of function, improve strength, flexibility, and range of motion. Our experienced physical therapists complete a thorough evaluation and will discuss your goals post-operatively in order to develop a treatment program best suited to your individual needs.

Patient and family education is also provided. While our therapists provide encouragement as you work toward your goals, commitment and active involvement also plays a key role in recovery. Because we understand the link between mobility and living a healthy lifestyle, our focus is to return you to the activities you enjoy.

 

 

 

 

IV Infusion

IV therapy, or infusion therapy, is the administration of intravenous fluids and medications. IV therapy can now be done in the comfort of your own home.

IV therapy is most often used for pain management, treatment of infections, chemotherapy and to manage other conditions. Our professional staff at Home Life Healthcare is available to administer your IV therapy and work closely with your doctor to adjust dosages & treatment if necessary.

Ostomy Management

At Home Life Healthcare, we offer knowledgeable and supportive nursing care to ostomy patients to help them regain their independence at home. Specially certified wound, ostomy and continence care nurses visit patients’ homes to teach them how to care for their ostomy and live comfortably.

 

 

 

 

Fall Prevention

Reducing your risk of falling is a critical way to help stay healthy and independent as long as possible. At Home Life Healthcare, we help reduce those risks. Our expert staff will come to your home to:

  • Evaluate any safety issues and provide suggestions on how to address them

  • Teach you how to get around safely both inside and outside your home

  • Help you with balance issues, using crutches, a walker, a cane, or a wheelchair

Physical Therapist
Develops and provides treatment programs for patients to restore function, relieve pain and prevent disability following disease, injury or loss of body part. Pain management can be given using ultrasonic therapy and T.E.N.S therapy.

 

 

Occupational Therapist
Assists and guides patients in the use of therapeutic, creative and self-care activities for the purpose of improving functions.

 

 

Speech Therapist

Provides rehabilitation services for speech and language disorders as well as swallowing impairments.

 

 

 

 

Medical Social Worker + Care Coordination

Medical Social Worker
